Steps to Register
Parking
:
Do
not
park in front of the pharmacy.
Use
plaza parking at the back.
If you park in front, you’ll be asked to move your vehicle
before receiving the form.
Clinic Visit:
Visit
Monday to Friday, 10:00 AM – 5:00 PM.
Bring the following:
Your
valid health card.
A
list of current medications
(print-out from your pharmacy).
Name and contact info
of your previous family doctor (if applicable).
Pre-Registration:
Fill out the form at the clinic.
You will then be
added to the waitlist.
The clinic will contact you
when it’s your turn
—there is
no set timeline
, so please be patient.
Meet and Greet Appointment
Arrive 15 minutes early
.
Bring your health card
If on regular prescription medications:
Bring a printed medication history
from your pharmacy.
Handwritten notes, bottles, apps, photos, receipts are NOT accepted
.
If not provided, you’ll be asked to
return with the proper document
.

Returning patients will have phone and online booking options after registration.
