New Patient Registration

Important Notes

Do NOT call the clinic or pharmacy for information. You must visit in person.
This clinic does NOT prescribe narcotics or controlled substances.
If you already have a family doctor, please continue with them.
Priority is given to patients in Peterborough who do not have a family physician.

Parking:
- Do not park in front of the pharmacy.
- Use plaza parking at the back.
- If you park in front, you’ll be asked to move your vehicle before receiving the form.
Clinic Visit:
- Visit Monday to Friday, 10:00 AM – 5:00 PM.
- Bring the following:
  - Your valid health card.
  - A list of current medications (print-out from your pharmacy).
  - Name and contact info of your previous family doctor (if applicable).
Pre-Registration:
- Fill out the form at the clinic.
- You will then be added to the waitlist.
- The clinic will contact you when it’s your turn—there is no set timeline, so please be patient.

Arrive 15 minutes early.
Bring your health card
If on regular prescription medications:
- Bring a printed medication history from your pharmacy.
- Handwritten notes, bottles, apps, photos, receipts are NOT accepted.
- If not provided, you’ll be asked to return with the proper document.

Dr.Tifase and Dr.Onifade are Family Physicians, offering rostered family practice and limited weekend community clinic for those without a doctor.
Returning patients will have phone and online booking options after registration.
For community health clinic
You must register in person and have a valid health card.
Patients will be seen by appointment only on a first come, first serve basis.
To check waitlist status, you must visit in person. No phone updates are provided.
