Northcrest Clinic with Dr. Tifase and Dr. Onifade offers family practice and limited walk-in services for both OHIP and non-OHIP patients during regular hours, after hours, and weekends—by appointment only.

We are currently accepting new patient registrations for both Family Practice and Walk-In services. If you are interested in becoming a patient, please visit our clinic in person to complete a registration form.

BOOK AN APPOINTMENT

PARKING:

Parking in front of the Pharmacy is reserved for Pharmacy patients only. Please follow instructions for the clinic parking area.

  • Do not park in front of the pharmacy.
  • Use plaza parking at the back.
  • If you park in front, you’ll be asked to move your vehicle before receiving the form.

How to get to the clinic:

The clinic is located at Northcrest Plaza with plenty of free parking. Entry to the plaza is through Hilliard St. and Marina Blvd. ( see map)

Follow the Arrow for entering the parking lot. When you get to the clinic, you will check-in with Shanda.
